Or can full-body training be just as effective for muscle growth? I’m going to answer all of these questions today and you may be a little surprised by some of these answers… So let’s begin with the pros and cons of each starting first with split training which is also referred to as “a bro split” and it’s usually the default type of training used by bodybuilders. With a very basic split training routine, you’ll be setting up your workout in a way where you’re focusing on only one muscle or one muscle group per day. So a typical basic split training routine might look something like this: Monday would be your chest day. Tuesday your back day. Wednesday would be your leg day. Then Thursday would be shoulders and abs, and then Friday would be biceps and triceps. Then Saturday and Sunday would be your two off days, and you would start the whole routine again on Monday by going back to chest. With full-body training, on the other hand, it's pretty obvious that you’re going be working your entire body every single session. That means unlike the bro split training you’ll be working your chest, back, arms, legs, and abs all in one day. Usually, a typical full-body training routine will have you working out three times a week with one day off in between each session. So Monday, Wednesday, and Friday would be full-body workouts and then the rest of the week you would have off. So which one of these is better for building muscle? Well, if we want to build muscle and increase our lean body mass faster we want to focus on increasing the volume of our workouts and volume is measured by 4 factors. Intensity, which is the amount of weight you’re lifting, frequency, which essentially is how many days a week you're working for a muscle group, and then there’s, of course, the total amount of sets, and reps that you’re performing. Increasing any of these 4 factors will help you build more muscle. Now immediately you may notice an advantage that full-body workouts provide over a basic split training routine. They require fewer days at the gym, only 3 instead of 5, and you’ll be targeting all the muscles in your body 3 times a week every week as opposed to split training where you’ll only be targeting each muscle once a week. That means that we’ll be increasing that frequency factor much higher with full-body training, and even if you do spend an entire workout session really beating up your chest with a split training routine, it shouldn’t take longer than a maximum of 3 to 4 days for your chest to recover, which means you’ll miss out on potential muscle gains, since you’ll be waiting 7 days to work your chest again instead of 4 days. So to make up for this lack of frequency other bro split routines have grown in popularity like push/pull routines, opposing body part routines, and upper/lower routines. A push/pull routine focuses on grouping your push muscles like your chest, shoulders, and triceps separately from your pull muscles like back and biceps. A typical routine may look like this: Day 1: Chest, shoulders, and triceps, Day 2: legs and abs, Day 3: Back and biceps, and Day 4 you would take off. Then you would repeat which would allow you to hit each muscle group twice a week. Now With an opposing body part routine, you would group two opposite muscle groups like your chest and back into one workout and each time you perform a set for your chest you would immediately perform another set with no break for your back. By super setting exercises like bench press and rows together you can still lift heavy for each exercise since each exercise works different muscles, and you can save time by pairing two sets together. An example routine would be Day 1: Chest and Back, Day 2: Would be a Leg day where you would be pairing quad and hamstring movements together and Day 3...

I'm Dr Holly Lofton. I'm an obesity medicine specialist at NYU Langone Health in Manhattan. Today I have a special guest, Dr Marina Kurian, who is a clinical professor of surgery at NYU Langone Health and the president of the American Society for Metabolic and Bariatric Surgery. Welcome, Dr Kurian. Marina Kurian, MD: Thank you so much for having me. Holly. Thank you. Lofton: Today, I want to discuss a very hot topic, which is the combined use of bariatric surgery and antiobesity medications, because we know there are so many different modalities that are being used to treat overweight and obesity right now. Those include lifestyle interventions, antiobesity medications, and bariatric metabolic surgery. However, some patients need multiple modalities to achieve success. I want to ask you first: What are some of the more common metabolic bariatric surgeries that are performed today? Kurian: I'd say the two most common currently in the United States are the sleeve gastrectomy and the Roux-en-Y gastric bypass. In the sleeve gastrectomy, we remove a portion of the stomach, whereas in the gastric bypass, we make the stomach small and then reroute the intestines — so we don't remove any stomach in that procedure. With the sleeve gastrectomy, we expect patients to lose about 30%-35% of total body weight. And with the bypass, it can be slightly more — maybe 35%-40% of total body weight. Lofton: Have you seen patients who don't achieve this expected weight loss? Kurian: Absolutely. And when that occurs, something that we both do in our practices is to add antiobesity medication to that regimen. But first, of course, I look to see what type of diet they're on, what their caloric intake is, and what is their level of activity before I just say, "Hey, take this medication." Lofton: What would you say are other potential benefits of using antiobesity medications after bariatric surgery vs a revisional surgery or a second metabolic or bariatric surgery? Kurian: It's a complex answer. Some of the benefits obviously are if the medication can get the patient to their goal, then we're saving them an operation. I frequently do this with my patients first to see if they really would benefit from a revision. I'll start them on medication after their initial bariatric surgery to see how well they do with dietary modification and behavioral modification. And then I add the medication as well. If they lose enough weight, they don't need a bariatric revision. But if they don't lose or there's some other reason — like sometimes it's about access to the medications — then I will consider them for a revisional procedure, depending on what their initial operation was, of course. Lofton: Is there scientific evidence that antiobesity medications are better for patients after bariatric surgery than, say, just seeing a dietitian to get more results? Kurian: Absolutely. As you know, and I believe you've written some of that literature, there are plenty of articles out there that look at the use of antiobesity medications vs lifestyle change and dietary changes. Antiobesity medication along with that will really have the maximum results for patients. This is if they have weight recurrence after the initial procedure, or if they have inadequate weight loss and they're not following the trajectory that I usually feel that they should be on — based on my experience with so many patients. Lofton: That's a really good point, and I want to talk more about that for the audience. If you're a primary care physician and you're seeing someone with a previous bariatric surgery, even if it was 5 or 10 years ago, a very common practice was to refer them to a dietitian and have them log and track calories and things like that. But if the evidence is stating that they may be better managed by either seeing their bariatric surgeon for medication or revisional options, or seeing an obesity medicine specialist or even the primary care doctor prescribing medications, if the evidence is there, then that may change some of the current practices and help our patients get more success. Kurian: I think that's absolutely true. If you look at the landscape of obesity treatments that are out there, because we've had so many medications added to our arsenal, I know that many of our colleagues around the country are getting more familiar with using them in patients. I'm Dr Holly Lofton. I'm a board-certified obesity medicine specialist, and I direct the medical weight management program at NYU Langone Health. With all the excitement about GLP-1 agonists, I get many questions from providers about which antiobesity drug they should prescribe. I'll tell you the methods that I use to determine which drug is best for which patient. Of course, we want to make sure that we're treating the right condition. If the patient has type 2 diabetes, we tend to give them medication that is indicated for type 2 diabetes. Many GLP-1 agonists are available in a diabetes version and a chronic weight management or obesity version. If a patient has diabetes and obesity, they can receive either one. If a patient has only diabetes but not obesity, they should be prescribed the diabetes version. For obesity without diabetes, we tend to stick with the drugs that are indicated for chronic weight management. Let's go through them. Exenatide. In chronological order of approval, the first GLP-1 drug that was used for diabetes dates back to exenatide (Bydureon). Bydureon had a partner called Byetta (also exenatide), both of which are still on the market but infrequently used. Some patients reported that these medications were inconvenient because they required twice-daily injections and caused painful injection-site nodules. Diabetes drugs in more common use include liraglutide (Victoza) for type 2 diabetes. It is a daily injection and has various doses. We always start low and increase with tolerance and desired effect for A1c. Liraglutide. Victoza has an antiobesity counterpart called Saxenda. The Saxenda pen looks very similar to the Victoza pen. It is a daily GLP-1 agonist for chronic weight management. The SCALE trial demonstrated 8%-12% weight loss with Saxenda. Those are the daily injections: Victoza for diabetes and Saxenda for weight loss. Our patients are very excited about the advent of weekly injections for diabetes and weight management. Ozempic is very popular. It is a weekly GLP-1 agonist for type 2 diabetes. Many patients come in asking for Ozempic, and we must make sure that we're moving them in the right direction depending on their condition. Semaglutide. Ozempic has a few different doses. It is a weekly injection and has been found to be quite efficacious for treating diabetes. The drug's weight loss counterpart is called Wegovy, which comes in a different pen. Both forms contain the compound semaglutide. While all of these GLP-1 agonists are indicated to treat type 2 diabetes or for weight management, Wegovy has a special indication that none of the others have. In March 2024, Wegovy acquired an indication to decrease cardiac risk in those with a BMI ≥ 27 and a previous cardiac history. This will really change the accessibility of this medication because patients with heart conditions who are on Medicare are expected to have access to Wegovy. Tirzepatide. Another weekly injection for treatment of type 2 diabetes is called Mounjaro. Its counterpart for weight management is called Zepbound, which was found to have about 20.9% weight loss over 72 weeks. These medications have similar side effects in differing degrees, but the most-often reported are nausea, stool changes, abdominal pain, and reflux. There are some other potential side effects; I recommend that you read the individual prescribing information available for each drug to have more clarity about that. It is important that we stay on label for using the GLP-1 receptor agonists, for many reasons. One, it increases our patients' accessibility to the right medication for them, and we can also make sure that we're treating the patient with the right drug according to the clinical trials. When the clinical trials are done, the study populations demonstrate safety and efficacy for that population. But if we're prescribing a GLP-1 for a different population, it is considered off-label use. I'm not gonna tell you to lie down on the ground and start doing crunches. In fact there's not gonna be a single crunch in this whole video because crunches and abdominal exercises are not what will help you lose belly fat. Let me repeat that crunches and ab exercises will never ever ever be the best exercises for you to lose belly fat. The exercises that I show you today are going to be far better and much more effective at burning fat at a faster rate. Before we get started there's a huge myth that I want to bust especially for the beginners watching this video. It's a myth that you can target a particular area of your body for fat burning. What I mean is that there's no exercise out there that will target specifically belly fat. No such exercise in the world. There are exercises that are better at burning overall body fat, and I'll go over those exercises with you today, but your body is going to pull from fat stores from all different areas. Also unfortunately medial areas of the body like the belly, butt, upper thighs, and love handles are last spots to go. You're likely to lose fat off your face before you start losing it from your belly. Don't worry though as you burn away more and more excess body fat, you eventually start burning fat from those problem areas, and that's when you'll lose the belly fat. The point is to lose belly fat you have to concentrate on losing overall body fat..that's all you can do, and these exercises are the best body fat burning exercises that I know of. So let's get started. As always I will be going over regressions at the end, so if you're a beginner and you can't do some of the exercises don't worry there will be alternatives later. For the first exercise all we need is a platform to step up on. You're going to face the platform sideways at a ninety degree angle, and step up with the foot closest to the platform. As you come up you want to explode off of that foot and hop up & over as you switch your feet. Repeat this 4 times and then immediately hit the ground and perform 4 mountain climbers. Make sure you jump your feet far enough in on the mountain climbers, remember we want to be climbing mountains not anthills. Repeat this cycle of 4 and 4 for 10 reps. You can make this exercise far more effective by adding some dumbbells to the movement. This should definitely get that heart racing. On the next one we're gonna do a mayweather stand up. They're actually called Maywhether situps but I said no crunches, so I'm sticking to it. This exercise has fat burning benefits not because there's a situp involved, but because it's working a lot of large muscles and getting your heart rate high. You're going to get your feet under something that will hold them down and hold a weight directly above your head. Swing the weight toward your feet as you situp, bring the weight out in front of you, and stand up. Squat and sit back down to the floor to repeat the movement for at least 10 reps. Moving on to the next one we have a real leg blasting exercise. You're going to start by lunging backwards and kneeling down on one knee. Bring your other knee back so that you're kneeling on both knees. The same foot that you used to Lunge backwards steps up into a forward lunge. Stand up and bring both feet together shoulder-width apart squat down and jump up as high as you can. As soon as you land backwards lunge now with the other leg and perform the exercise on the other side. If it gets confusing and feels like too many steps you can just stick to doing one side at a time until you get the hang of it. Next we have one that looks really complicated but once you get it its a piece of cake. Try to take it Step by step and remember if you cant do it there will be regressions at the end. Start standing and perform a sprawl.
